BEFORE FILLING IN THIS FORM... you will need to DESTROY 1 FAKEWAX.REDEEM token for each record you would like to claim. For most of you, this will mean 1 Token for 1 Record. IF YOU HAVE MULTIPLE TOKENS, please destroy them in one batch so we can more easily track. You will need to destroy before filling out the form below, as the system will automatically search for the destruction transaction based on the XCP address you provide. You will also want to wait until the transactions have confirmed.

We will only use the email and phone number provided for any potential shipping issues... we're not trying to doxx anybody, we just want to share this little piece of history with you.
